Signature Smiles Premium Savings Plan – $400 Per Patient Per Year

Services included are as below:

  • 2 Cleanings per year (Either D1110 or D4910) Co-Pay of $50
  • 2 Exams per year
  • 2 X-rays per year
  • 2 Fluoride/FYGG Treatments per year

Any additional services will be discounted 15%. Plans begins the date of purchase for a plan year. All services must be completed within that time frame and any services not completed are forfeited and not eligible for refund or credit. Savings Plan cannot be combined with use of Care Credit or Insurance. Cannot be transferred or substituted.

The “Welcome to Our Office” Special – $109

Includes necessary x-rays, complete doctor examination, oral cancer screening, tobacco counseling, and oral hygiene demonstration.

Emergency Special – $99

Includes a limited doctor examination and 1 PA X-Ray.

Complimentary Second Opinion Consultation

Must bring your own X-rays along and present them to the doctor at the visit.
